Every payment limit row from every CMS file loaded, 1Q21 to 4Q26, covering 1,205 HCPCS codes from 24 CMS files. When CMS re-posted a revised file for a quarter, rows from every version are included, and is_current marks the newest.

Columns

effective_quarter

Quarter the payment limit applies to, e.g. 2026Q4.

hcpcs_code

HCPCS code.

short_description

CMS short descriptor, exactly as in the file.

hcpcs_dosage

HCPCS code dosage (the billing unit the limit is for).

payment_limit

Payment limit in dollars. Blank where CMS gave no limit.

payment_limit_as_published

The payment limit text exactly as it appears in the CMS file (for example N/A).

source_file

The CMS ZIP file the row came from.

source_url

Where CMS published that file.

cms_posted_date

Date CMS posted that file, where the CMS page lists one.

file_version

1 for the first file posted for a quarter, 2 for the first revision, and so on.

is_current

true for rows from the newest version of each quarter's file.
